In accordance with the Rental Agreement, if you do not purchase optional Loss Damage & Theft Waiver, you must provide Contractors Equipment Center with proof of insurance or a risk floater naming Contractors Equipment Center as loss payee on any and all equipment rented. If you do not purchase optional Loss Damage & Theft Waiver, you may be responsible for the full cost of any and all repairs or the full replacement cost of the equipment. The waiver is subject to the exclusions in the Rental Agreement. The Loss Damage & Theft Waiver fee is Fourteen (14) percent of the rental charges and is also subject to a minimum deductible as outlined in the Rental Agreement.
Open Account Credit Terms
1. Each invoice is due and payable in full within 30 days from the date of the invoice.
2. If equipment is rented for more than 4 weeks, periodic unsigned invoices will be issued for rental charges due. All such invoices are due and payable in full within 30 days of the invoice date.
3. At the discretion of Contractors Equipment Center, any account with a delinquent balance will incur a service charge and may be placed on a cash basis at anytime, and the equipment picked up without notice.
4. Contractors Equipment Center files preliminary lien notices and mechanics' liens whenever necessary or required by law. This is company policy, and is not a reflection of your credit standing or rating. The undersigned agrees to furnish Contractors Equipment Center with accurate information necessary to complete the notices and liens.
5. Customer agrees to pay on each delinquent account a monthly service charge of 1 ½% of the amount due or the maximum permitted by law in the state where the contract is signed. A fee of $25 will be charged for all N.S.F. Checks.
6. Customer agrees to pay all reasonable attorneys fees, collection and court costs incurred by Contractors Equipment Center in enforcing the Rental Agreement and these terms and conditions.
7. Customer, and guarantor if applicable, authorizes Contractors Equipment Center to obtain credit reports, trade reports, and bank references for the purpose of determining the extension or continuation of credit.
8. If there is a conflict with the provisions of this Credit Application and the Rental Agreement, the provisions of the Rental Agreement shall control.

It is understood and agreed that you have specifically consents to Contractors Equipment Centers investigation of the applicant's credit history and may utilize credit reporting services to obtain information about the undersigned on an ongoing basis. Facsimile copies will be accepted as originals.
Continuing Personal Guaranty
By submitting this web form, you hereby unconditionally guarantee(s) the full and prompt payment to Contractors Equipment Center when due of any and all indebtedness, obligations, and liabilities of the customer named in this Credit Application, including all amounts now owing and arising in the future, and including any interest, attorneys fees, and collection and court costs. You also agree to be personally bound by all terms of this Credit Application and each Rental Agreement. It is understood and agreed that you specifically consents to Contractors Equipment Centers investigation of the applicant's credit history and may utilize credit reporting services to obtain information about you or your company on an ongoing basis. This guarantee shall continue in force until notice in writing sent by certified mail, return receipt requested, is received by Contractors Equipment Center. This notice shall specify the date of termination, not to be less than seven (7) days after the notice is received and shall not affect any charges for transactions with the customer that were entered into prior to the termination date.
